Requirement

  • Candidates must be licensed in at least five of these states:
    • AR, CO, IA, ID, IN, LA, MI, NE, OK, TN, TX, WY, NV, AK, AZ, CA, HI, IL, MN, MT, NM, OR, UT, WA, MO
  • *IF* candidate is licensed in one of the above State, they must hold IP in that state. If they don’t hold a license in one of these states, they don’t need IP.
  • Arkansas (AR): 6240 hours of experience.
  • Connecticut (CT): 2000 hours and 3 years with any collaborating physician (CP); other state collaborating hours qualify.
  • Florida (FL): 3000 hours of overall experience (100 weeks or 2.5 years).
  • Illinois (IL): 4000 hours (133 weeks or 3 years of overall practice).
  • Kentucky (KY): 4+ years of overall prescribing experience.
  • Massachusetts (MA): 2+ years of experience.
  • Maine (ME): 24 months of supervised practice.
  • Minnesota (MN): 2080 hours of experience.
  • Nebraska (NE): 2000 hours of overall experience.
  • New York (NY): 3600+ hours of overall experience (120 weeks or 2.5 years of practice).
  • South Dakota (SD): 1040 hours of experience.
  • Virginia (VA): 3 years of overall experience.
  • Vermont (VT): 2400 hours of overall experience.
  • West Virginia (WV): 3 years of overall prescribing experience.

Responsibilities

  • Evaluate and manage primary care, acute, chronically ill, and transitional care patients, providing healthcare education, counseling, and disposition planning for patients aged 18 months and above.
  • Provide patient counseling on pregnancy prevention, STI prevention, safer sex practices, contraceptive care, and medication management.
  • Educate patients on health maintenance and address patient care inquiries.
  • Document all patient care in the EHR in compliance with policies and procedures.
  • Coordinate patient care with internal and external colleagues, including the broader patient-centered medical home, ensuring the highest standard of care at all times.
  • Collaborate effectively within a patient care team, including fellow providers, collaborative physicians, para-professionals, pharmacists, and other healthcare team members.
  • Must be available to work two weekends per six-week cycle.
  • Must be available to work one major holiday and one minor holiday per year.
  • Work up to 20 hours per week; exact hours TBD. Current business hours are 6 AM to 11 PM, including weekends.
